چکیده

Abstract Background Several factors have been associated with severity of COVID-19 disease, but there remains a paucity data surrounding whether the nature exposure is impactful. Evidence demonstrating correlation between initial viral dose and disease exists for many infections. Observational studies suggested that context, which can be considered proxy magnitude inoculum, may influence infection. We aimed to assess having known exposure, as higher inoculum COVID-19, was more severe outcomes individuals hospitalized COVID-19. Methods created retrospective cohort community-dwelling adults in south-central Ontario from April 1, 2020 - January 14, 2021. Individuals or next kin were contacted ascertain history. The primary outcome death, intensive care unit (ICU) admission, mechanical ventilation (MV) within 30 days admission. A multivariable logistic regression model used determine worse outcomes. Results 1097 community acquired required hospitalization; these, 942 (86%) had available data. In this group, median age 65, 44% women, 84% lived private residence, 59% frailty score (FS) 1 – 3 while 40% FS 4 9, 28% exposure. Overall, occurred 368/942 (39%) patients. Having not (OR 1.14, 95% CI 0.84–1.54, p = 0.41). Male gender 1.41, 1.06–1.89; 0.018), 1.01/year, 1.00–1.03, 0.03), 1.22/point, 1.09–1.36, 0.001) living at least one other person 1.57, 1.09–2.28, 0.017) all ICU MV Conclusion While outcome, identified increased illness cohabitation suggests context role severity. Over-stimulation of the immune system followed by excessive pr...
